Get back in motion. Orthopedic, sports, and regenerative medicine physicians in Bethesda, MD

Knee, shoulder, back, hand, or foot pain? The fellowship-trained physicians at Propel Sports and Orthobiologics find the source with in-office ultrasound, then treat it with regenerative medicine, advanced procedures like Tenex, and shockwave therapy, so surgery is rarely the first answer.

Physicians and physician assistants, one plan

Fellowship-trained physicians lead every plan and perform every procedure. Physician assistants help you get seen sooner and keep your recovery on track.

Orthopedic care, without the operating room.

Interventional orthopedics

Interventional orthopedics treats tendon, joint, ligament, hand, and spine problems with precise, image-guided procedures instead of open surgery. Recovery is usually faster, and most treatment happens right here in the office.

Ultrasound-guided procedures

Needles and small instruments guided to the exact injured structure: tenotomy, releases, hydrodissection, and nerve procedures.

Regenerative medicine

Orthobiologics such as PRP, bone marrow aspirate concentrate, and Lipogems that harness your body's own ability to heal.

Shockwave therapy

Focused sound-wave pulses, with no needles or downtime, to restart healing in chronic tendon and heel pain.

See the problem. Treat it precisely.

Ultrasound shows tendons, ligaments, joints, and nerves in real time, often while you move, so a diagnosis can come at your first visit. The same probe then guides every injection and procedure to its exact target.

Ultrasound-guided

Carpal Tunnel Release

Releases the ligament pressing on the median nerve through a tiny incision under live ultrasound, without open surgery.

Ultrasound-guided

Trigger Finger Release

Releases the pulley that catches a locking finger, through a needle under ultrasound.

Ultrasound-guided

Guided Injections

Office-based injections placed under live ultrasound, so the medicine reaches the exact structure that hurts.

Ultrasound-guided

Orthobiologics (PRP)

A concentrate of platelets from a small draw of your own blood, placed precisely under ultrasound to calm pain and support tendon and joint healing.

Your path back

Four steps from your first visit to doing what you love again.

  1. Evaluate

A real conversation about what hurts and what you want back, then a hands-on exam.

  2. See it on ultrasound

In-office ultrasound shows the tendon, joint, or nerve involved, often at the first visit.

  3. Treat the source

Guided procedures, orthobiologics, shockwave, or rehab: the least invasive option that can work.

  4. Return and propel

A strength-first plan with clear milestones gets you back to training, work, and sport.

The goal is to keep you moving

Less time on the sidelines, more time doing what you love.

  1. Symptom controlWe calm pain first, with targeted injections or nerve care when needed, so you can sleep, move, and train.
  2. MobilityWe restore the motion your sport, job, and daily life ask for.
  3. StrengthProgressive strength protects joints and tendons and keeps problems from coming back.
  4. RecoverySleep, training load, and nutrition shape how you heal, so we plan for the whole person.

What patients say

“Dr. Gruner has performed procedures over the last year on my hands to cure trigger finger on 4 different fingers. He employs a needle/knife tool to perform a minimally invasive 20 minute or so procedure that heals in 3-4 days as opposed to using a scalpel employed by most hand doctors to rectify trigger finger that makes an incision that takes 2+ weeks to heal. He is one of only a few hand doctors in the DC area that use a needle/knife to rectify trigger finger. I highly recommend Dr. Gruner for rectifying trigger finger which is so common, particularly among older adults.”

“The experience of seeing Dr. Gruner was one the most positive interactions I have had with a medical professional. He was personable and provided a clear diagnosis based on evidence. He outlined various treatment options, their respective timeline, and comparative likelihood of resolving the issues. He was thoughtful in response to questions and was interested in making sure I understood what we discussed.”

“This was my first visit with Dr. Gruner. He was friendly, professional and easy to talk to about my knee problems, including past issues. He thoroughly explained all available options to address my pain and answered my sometimes repetitive questions. His assistant, Mary Kate, also was very helpful during the appointment and procedure. Hope to work with them both during follow-ups. Overall, I was completely satisfied.”

“I was given an ultrasound cortisone shot in my hip on Monday, Feb 16. At that time i was not able to walk upstairs one foot after the other. On Friday morning, Feb 20 - 4 days later - I could walk upstairs one foot after the other! Now, two weeks later, I continue to be able to use both legs while climbing stairs and have regained much of my previous muscle use; long walks with even slight inclines are a challenge - but a minor one! I also made contact with Sally Miller, of Limberhealth, whom I was strongly encouraged to use by Dr Gruner. Ms Miller did a terrific job of orienting me to the PT materials. Today in the Bethesda office, Mary Kate did an excellent job in orienting us as we awaited Dr Gruner.”

“I’ve been dealing with knee pain for over 2 years, and I finally got it checked out with Dr. Gruner under the ultrasound. He immediately saw what’s going on without me needing to go do an MRI, and he suggested me to do a PRP with him. The PRP was really simple and pain free, and he regularly checked up on me to make sure I was doing okay. I am now pain free and really excited to get back to playing sports again!”

Common questions

What kind of doctor should I see for joint or muscle pain?
For most shoulder, elbow, hand, hip, knee, foot, and back pain that is not an emergency, a sports medicine or interventional orthopedic physician is a good first stop. At Propel we find the source with an exam and in-office ultrasound, treat most problems without surgery, and refer to a surgeon when surgery is the better choice.
Does Propel offer regenerative medicine?
Yes. Our physicians offer PRP, PRP with protein concentrate, bone marrow aspirate concentrate, microfragmented adipose tissue (Lipogems), and prolotherapy, each placed under ultrasound and paired with rehab. They can ease pain and support tendon and ligament healing, though they do not regrow cartilage.
What is interventional orthopedics?
Interventional orthopedics treats orthopedic injuries and arthritis with precise, image-guided procedures instead of open surgery: ultrasound-guided injections, orthobiologics, and minimally invasive releases performed in the office. At Propel it is paired with shockwave therapy and strength-based rehab.
What are orthobiologics?
Orthobiologics are treatments prepared from your own tissue: platelet-rich plasma (PRP) from your blood, bone marrow aspirate concentrate (BMAC), and microfragmented adipose tissue (MFAT) from a small amount of fat. Placed precisely under ultrasound, they can reduce pain, improve function, and support healing of tendons, including small partial tendon tears.
Can PRP or other orthobiologics regrow cartilage?
No. Orthobiologics do not rebuild worn cartilage. For some people with arthritis, PRP and other orthobiologics can ease pain and improve function, which can help them stay active and delay surgery. Your physician will explain what a treatment can and cannot do for your joint before you decide.
What is shockwave therapy?
Extracorporeal shockwave therapy (ESWT) delivers focused sound-wave pulses through the skin to stimulate healing in chronic tendon and heel pain, such as plantar fasciitis, Achilles tendinopathy, and tennis elbow. There are no needles or downtime, and it is paired with a strengthening program.
Does Propel treat hand and wrist problems?
Yes. We perform ultrasound-guided carpal tunnel and trigger finger releases in the office, and treat thumb arthritis, de Quervain's tenosynovitis, and hand tendon pain with ultrasound-guided injections, orthobiologics such as PRP, and hand-focused rehab.
Who will I see at Propel?
You will see a fellowship-trained Propel physician or a physician assistant working with one. If you are unsure which specialist fits your problem, book any appointment and we will route you.
Why do Propel's physicians use ultrasound?
Ultrasound shows tendons, ligaments, muscles, joints, and nerves in real time, including while you move, so your physician can often confirm a diagnosis at the first visit. It also guides every injection and procedure to its exact target instead of relying on landmarks alone.
Does insurance cover treatment at Propel?
Office visits, diagnostic ultrasound, and many procedures are billed to insurance. Most insurers do not currently cover orthobiologic injections such as PRP, BMAC, and microfragmented adipose tissue (MFAT). We review costs with you before any treatment.
When does Propel Sports and Orthobiologics open, and can I book now?
Propel Sports and Orthobiologics opens on December 1, 2027, and we are scheduling new patients now. Book online or call (301) 200-9025 to reserve an appointment.
Where is Propel Sports and Orthobiologics located?
