Co-Founder · Sports Medicine and PM&R

Dr. Marc Gruner co-founded Propel Sports and Orthobiologics to give active people in the Washington, DC region a clear answer about what is causing their pain and a plan that keeps them moving. He trained in Physical Medicine and Rehabilitation at Georgetown University and the National Rehabilitation Hospital, then completed a sports medicine fellowship at Mayo Clinic in Rochester, Minnesota, where he concentrated on orthobiologics and diagnostic and interventional ultrasound.
Ultrasound sits at the center of his practice. He uses it at the exam table to see tendons, ligaments, nerves, and joints in motion, and to guide every needle he places. That precision supports a range of office-based procedures: platelet-rich plasma, bone marrow aspirate concentrate, and microfragmented adipose tissue injections, percutaneous tenotomy for stubborn tendons, tendon scraping, and ultrasound-guided releases for carpal tunnel syndrome, trigger finger, and chronic exertional compartment syndrome.
His work reaches beyond the clinic. Dr. Gruner co-authored the Remote Therapeutic Monitoring CPT codes through an American Medical Association workgroup, served as a physician advisor at the CMS Innovation Center (CMMI), and has contributed to national quality measurement as a member of a PQM committee. He co-founded Limber Health, a musculoskeletal value-based care company, and served as its Chief Medical Officer. That experience shapes how he practices: measure what matters, and spend care where it changes outcomes.
He holds a joint Doctor of Osteopathic Medicine and MBA from the Edward Via College of Osteopathic Medicine and Virginia Tech. A Bethesda native, he lives in the DC area with his wife and daughter and spends his free time cooking, hiking, and traveling.

We listen first
Your visit opens with a real conversation: how the problem started, what you have already tried, and the activity you want back.
Exam and in-office ultrasound
A hands-on exam, then ultrasound at the exam table, often while you move, so your physician can see the tendon, joint, or nerve involved.
A clear diagnosis and plan
You leave knowing what is wrong and what your options are, from strength-based rehab to procedures, and why we recommend each one.
Treatment and follow-through
Most procedures happen here in the office. We coordinate with your physical therapist and check progress against the goals you set.
Opening December 1, 2027
We are scheduling new patients now. Book an evaluation to find out what is causing your pain and how to treat it.
